How do birth abnormalities of the uterus affect pregnancy? In this video, Mark P. Trolice, M.D., board-certified reproductive endocrinology and infertility specialist and founder of Fertility CARE: The IVF Center shares his insights on the different types of congenital irregularities he commonly sees and what to do about them. “Congenital abnormalities of the uterus don’t typically cause infertility, but they can cause problems during pregnancy,” says Dr. Trolice. “The incidence is probably about 4% of women are born with a uterine abnormality.” Dr. Trolice explains the process of embryology - how the uterus is formed in the female fetus while in the womb. “If the uterine cavity did not fuse properly in the womb then a woman can end up with a bicornuate uterus,” he says. “It doesn’t cause infertility but the malformed uterus can increase the risk of miscarriage, preterm labor and delivery, and breach thereby necessitating a cesarean section.” “Another congenital uterine abnormality is called a didelphys where a woman has two cervixes and two uteri,” Dr. Trolice continues. “That has no consequence other than preterm labor and breach, as both half-uteri are complete in their own right.” “The abnormality we see most commonly in patients who have recurrent miscarriage is a septum representing 10-15% of patients. The top of the uterus is fine, but it didn’t core out. Instead of normal endometrium, you have fibrous tissue increasing the risk of miscarriage to about 44%.” According to Dr. Trolice, bicornuate uterus and didelphys are not surgically treatable and should not be treated. However, a septum can be treated with a minor outpatient procedure. “We put a hysteroscope inside the uterus, burrow a hole through the fibrous tissue, and open it up until you have a normal uterine cavity,” explains Dr. Trolice. “A mild septum called an arcuate uterus is when the septum is less than 1 cm, and nothing needs to be done. There are variations on all these conditions. If you have a one-sided uterus, called a unicornuate uterus, sometimes you will find a residual horn (on the other side). If it is not communicating with the cervix and the vagina, then you can have a normal uterine cavity within the horn, and become pregnant in there. Now that can be dangerous as the fetus has nowhere to go (known as an ectopic pregnancy). The horn, unfortunately, needs to be surgically removed.” However, if no horn is present, there is no risk of conceiving and carrying a baby to term within a one-sided uterus. “The bottom line is that there is no urgency with any of these situations,” concludes Dr. Trolice. “None of these irregularities should prevent you from being able to conceive.
