How an Ultramarine is created involves 19 Space Marine gene-seed implants, years of surgery, and a Space Marine creation process that kills most who attempt it. Every candidate undergoes tissue compatibility testing and genetic screening before a single implant is introduced. Only a small percentage pass. The gene-seed is keyed specifically to male hormones and genetic structure, meaning most humans are incompatible before the process even begins. The 19 implants are introduced in stages across several years. Phase 1 installs a secondary heart capable of maintaining full life functions even if the original is destroyed. Phase 2 and 3 fuse the ribcage into solid interlocking bone plates and drive extreme muscle growth across the entire body. By Phase 8, the Omophagea allows a Space Marine to absorb genetic memory from consumed tissue. Phase 12, the Sus-an Membrane, enables suspended animation when the body sustains catastrophic damage. The final implant, the Black Carapace, connects the warrior's nervous system directly to his power armour. Many of the surgeries are performed without anaesthetic.
