Its time to move on from CRSwNP and CRSsNP! Its been a long time since our profession moved from an established classificaiton system. However, changing the way that we discuss chronic rhinosinusitis (CRS) is essential if we are to integrate our understanding of the disease and the emerging therapies into optimal clinical care. The working classification system for CRS needs to move away from the CRSwNP and CRSsNP dichotomy. With our scientific advances in the understanding of CRS, these classifications need to change in order to change clinical practice. Guidelines and management schema have been reticent to change from the historical description of CRS. This proposed system based on anatomical distribution and endotype/inflammatory dominance, is simple and practical, and has been adopted by the European Position Paper on Rhinosinusitis 2020 (EPOS2020). While research continues to further define the subtypes of CRS into phenotypes and endotypes, the proposed classification system of primary CRS by anatomical distribution and endotype dominance allows for a pathway forward. https://www.rhinologyjournal.com/Docu...
