Synovial Chondromatosis Synovial chondromatosis is a rare, non-cancerous condition affecting the synovium (the lining of joints). It is characterized by the formation of cartilage nodules in the synovial membrane, which may detach and form loose bodies within the joint. This can lead to pain, swelling, and reduced joint mobility. --- Key Features 1. Pathophysiology: Abnormal growth of cartilage in the synovium. Nodules may calcify and form loose bodies in the joint space. Commonly affects larger joints such as the knee, hip, elbow, or shoulder. 2. Types: Primary Synovial Chondromatosis: Idiopathic, occurs without an underlying cause. Secondary Synovial Chondromatosis: Associated with joint trauma, osteoarthritis, or other inflammatory conditions. 3. Demographics: More common in individuals aged 30–50 years. Slightly more frequent in men than women. --- Symptoms Gradual onset of joint pain and swelling. Reduced range of motion. Clicking, locking, or grinding sensation in the joint. Sometimes effusion (fluid in the joint). Symptoms may worsen with activity and improve with rest. --- Diagnosis 1. Clinical Examination: Joint tenderness, swelling, and decreased mobility. Palpable loose bodies in some cases. 2. Imaging: X-rays: May show calcified loose bodies if they have ossified. MRI: Useful for identifying cartilaginous nodules and evaluating the extent of synovial involvement. CT scan: Provides detailed views of calcified bodies. 3. Histopathology: Biopsy may be done to confirm the diagnosis and rule out malignancy (e.g., chondrosarcoma). --- Treatment Treatment aims to relieve symptoms and prevent joint damage. 1. Non-Surgical: Pain management with NSAIDs. Physical therapy to maintain joint mobility. Reserved for mild cases or those with limited symptoms. 2. Surgical: Arthroscopic Synovectomy: Removal of loose bodies and diseased synovium. Minimally invasive and effective for early cases. Open Synovectomy: Required for extensive involvement or large joints. In advanced cases with severe joint damage, joint replacement may be considered. --- Prognosis Primary Synovial Chondromatosis may recur after surgery, so long-term follow-up is necessary.
