Dear All, In this video, we share pearls for topical phacoemulsification & Tecnis Toric IOL (21.0 Diopter, ZCT 300) Implantation in a Post Radial Keratotomy (RK) Cataract Case with 2.50 Diopter of corneal astigmatism. Post radial keratotomy (RK) cataract cases present several challenges to the refractive cataract surgeon. The IOL power calculations can be inaccurate, the surgical procedure can be challenging, and the postoperative recovery can be prolonged with fluctuation in visual acuity. Toric IOL Implantation in post RK cataract cases is helpful to reduce the corneal astigmatism. The surgeon should pay attention to all steps of cataract surgery- incisions, well centered CCC, hyrodissection and nuclear emulsification to ensure intact capsular bag/zonules. Removal of OVD behind the capsular bag is important to minimize the post op rotation of toric IOL. Ensure proper sealing of main incision and side-port incisions to avoid shallowing of the anterior chamber. The surgeon should counsel post RK patients undergoing cataract surgery with premium IOLs explaining to them that their IOL calculations are, at best, estimations and possibility of glasses for near and distance and/or undergoing another surgery (e.g. piggyback IOL implantation) to overcome refractive surprise.
