Multi-ligament injuries of knee have been treated with different protocols due to less prevalence as compared to single ligament injuries. Multi-ligament injuries are a result of collision sports or high energy trauma/ Accidents and usually has associated : 1. Fracture Around knee 2. Joint dislocations 3. Cartilage injury 4. Meniscal root tears 5. Bone Avulsion fracture of ligaments These should be dealt seriously because they might lead to consequences including amputation of the involved limb and severe dysfunction if not taken care of. It is also imperative to seek emergent medical/ surgical intervention in these cases. Most cases involving only soft tissue injury can be conserved for 6-8 weeks after addressing and ruling out all the injuries. In cases where there are concomitant injury to other knee structures, it becomes an emergency to operate on these joints to save the future complications. To address the need for surgery, careful clinical examination and history is imperative along with radiological investigations such as X-rays/ Skiagram, CT scan, MRI scan etc. It is also advised that the surgical procedure (if need be) to address ligament and soft tissue injury should be planned meticulously after 6-8 weeks of initial insult. The delay is advised so as to prevent arthrofibrosis/ Stiff knee post surgery and have excellent functional outcomes at 6-8 weeks after surgery. Rest post surgery is imperative and patients are allowed to bear weight as per protocol of repaired ligaments. Various authors have documented excellent results with variable physiotherapy patterns in these cases. With higher chances of complications such as : 1) Infection 2) Venous thrombosis 3) Pain 4) Swelling 5) Stiffness 6) Delayed rehabilitation treating these injuries becomes more complicated. It is advised to start passive physiotherapy early on to recruit the knee, hip and leg muscles.
