Morphea is a rare immune disorder characterized by skin sclerosis and atrophy of soft tissues. Autologous fat grafting has been used to repair a variety of soft tissue defects. However, due to severe soft tissue depression and skin sclerosis, patients with morphea often require multiple fat graftings. Cryopreserved stromal vascular fraction (SVF) gel, an adipose-derived product that concentrates SVF cells, could be convenient and beneficial for multiple injections. This study aimed to evaluate the safety and effectiveness of sequential fat grafting with fresh fat and cryopreserved SVF gel for morphea. This video provides a demonstration of the surgical technique of autologous fat harvesting and the procedure of SVF gel preparation and cryopreservation. Briefly, Coleman fat was harvested by centrifugation of lipoaspirates at 1200g for 3 minutes. A portion of the Coleman fat was mechanically emulsified by shifting between 2 syringes connected by a female-to-female Luer-Lock connector. The emulsified Coleman fat was centrifuged at 1600g for 3 minutes, and the sticky substance under the oil layer was defined as the SVF gel.
