It begins at the pylorus of the stomach and extends about 5 cm to the superior duodenal flexure. While its anterior relations involve the liver, gallbladder, and peritoneum, the posterior relations are exceptionally high-yield because they involve vital vascular structures and are often implicated in surgical and pathological complications 🧠🩸. The posterior relations of the 1st part of the duodenum include: • Gastroduodenal artery — this lies immediately behind and is vulnerable in posterior duodenal ulcers, which can cause massive upper GI bleeding 💉🔥 • Common bile duct (CBD) — crosses posteriorly and can be compressed or involved in inflammatory conditions like cholangitis 🟢🧪 • Portal vein — lies behind and slightly superior; important in hepatic and portal surgeries 💙 • Inferior vena cava (IVC) — deeper and posterior, lies more medially; critical relation for retroperitoneal access • Pancreatic head — lies behind the duodenal loop, often fused with the second part 🧬 These posterior structures are all retroperitoneal and closely packed, making this area extremely significant in surgeries like Whipple procedure, trauma cases, and perforated ulcer management ⚕️.
