SETTING: Emergency Department NURSE You see the parent of a four-year-old girl who has been treated in the Emergency Department for an undisplaced fracture in the middle third of her clavicle (collar bone). She has had a sling put on and is now ready to be discharged. You are giving the parent advice on aftercare and the healing process. The child is not present for the discussion. TASK • Confirm reason for seeing parent (advice on child's aftercare/healing process). Find out how parent is feeling about child going home. .Give information about use of sling (e.g., day and night, removal for bathing, extra pillows for arm support, etc.). Recommend pain medication for first few days (e.g., paracetamol/acetaminophen, before bedtime then as required, etc.). • Suggest ways of encouraging child's sling use (e.g.. customisation/decoration. slings on soft toys, etc.). Advise on cessation of sling use (e.g., usually 3-6 weeks, fracture site pain-free, ability to move shoulder/arm, etc.). •Stress importance of no rough play for 8-12 weeks after sling removal (e.g., bone still weak/fragile, risk of further fracture, etc.).
