Organ transplant requires removing organ and storing it without blood supply Cold ischemia time is the period between when an organ is cooled after removal (with reduced or stopped blood supply) and when it's rewarmed during transplantation. Times for different organs (variable by source): Kidneys: ~24 hours (up to 72 hours with perfusion pump) Liver: 6-10 hours Heart: 4-6 hours Lungs: 4-6 hours Why different times? Possibly related to organ's metabolic requirements and cellular composition. The kidneys have longest tolerance because: 1. lower metabolic demands compared to others 2. structurally more resilient to ischemia due to their tissue composition 3. maintain basic cellular integrity even with reduced oxygen and nutrients In contrast, heart and lungs: 1. very high metabolic demands (constant activity) 2. Contain tissues that are extremely sensitive to oxygen deprivation 3. Suffer more rapid cellular damage when deprived of normal blood flow
