Various guests from all over India and the world were invited to share their expertise and experience on different head and neck cancers. Speaker: Dr. Anil K D’Cruz Topic: How managing thyroid surgery complications has changed over the years In this video, the keynote speaker, Dr. Anil K D’Cruz explains how new ways of handling thyroid surgery complications are helping patients recover better and live healthier lives. Thyroid surgery has come a long way since the 1800s — and today, although complications exist, it’s safer and more effective than ever. Learn about what risks still exist, how doctors are working to prevent them, and why proper preparation and experienced surgeons make all the difference. In this video Dr. Cruz talks about: a. How thyroid surgery has become safer and more convenient over the years Mortality rates in thyroid surgery have dramatically decreased from 40% in the 1850s to near zero today due to advancements like Lister's antisepsis, antibiotics, and the meticulous techniques pioneered by surgeons like Theodor Kocher. While the incidence of some complications remains, the understanding and management have evolved significantly since the synthesis of thyroid hormone. b. The most frequent complications and how they impact everyday life The major distressing complications of thyroid surgery include voice changes (due to recurrent and superior laryngeal nerve injury) and hypoparathyroidism, both of which can severely impact a patient's quality of life. Recurrent laryngeal nerve injuries are a leading cause of medical malpractice suits, while hypoparathyroidism can necessitate lifelong calcium supplementation. c. How new tools and techniques help protect the glands and the voice of the patient Intraoperative nerve monitoring (IONM) is beneficial in identifying and safeguarding the recurrent and external branches of the superior laryngeal nerves during surgery. Techniques like meticulous dissection, low bipolar cautery settings, and the potential use of autofluorescence for parathyroid identification aim to minimize damage to these vital structures. d. What patients and families should know before and after surgery Proper preoperative counseling about potential complications, including the possibility of temporary voice changes and hypoparathyroidism (even in experienced hands), is crucial to manage expectations and avoid legal issues. Patients should also be aware of the importance of identifying potential advanced cancers with imaging for large or suspicious nodules. e. Why surgical experience and ongoing care make a difference Surgeon and hospital volume are significant factors in positive thyroid surgery outcomes, with higher volume surgeons generally having lower complication rates. Continuous intraoperative nerve monitoring, while debated, may offer benefits, and ongoing meticulous surgical technique, coupled with appropriate use of technology, are essential for optimal results.
