. . Removal of a facial dermal implant. This lady had this placed decades ago and now wants it removed. . A dermal piercing is anchored into the skin with a stud. The two things to keep in mind when removing a dermal in the location is to: 1. Keep the incision as small as possible to minimize the scar, and 2. Close the incision nicely to minimize the scar. . I inject the skin around the dermal piercing with 1% lidocaine and 0.25% bupivacaine with epinephrine. The lidocaine acts almost instantaneously to numb the skin. The bupivacaine is a long lasting local anesthesia. The epinephrine is to let the anesthesia last longer. It has nothing to do with minimizing blood loss. . The stud is carefully dissected out and the piercing is removed completely. The incision is too small to place any suture under the incision. I didn’t want to leave any sutures on the outside as they will leave suture hole scars. So I pulled the skin edges together and glued it shut so it will heal nicely without any suture. The only negative part is she will have blue glue on her face for two weeks.
