Age of Circumcision || Dr. Jayanta Bain Plastic and Cosmetic Surgeon Definition: Phymosis is a condition when the foreskin is less elastic and it cannot be retracted beyond the glans penis during erection or in even during flaccid condition. Age: This condition can be seen among all age of male. However, most patients presents between 15 years -35 years of age, when they become sexually active. Complications of Phymosis: Phymosis patient may present with different symptoms. i. Small meatus: This is a condition, the opening in front the foreskin is very much inelastic and contracted that it cannot be pulled back even when the penis is flaccid. The foreskin may swell like a balloon during urination. ii. Painful erection: For some patient, the foreskin becomes a tight ring after erection, and feels much pain to pull back the foreskin. This may lead to inability to masturbate or do coitus. iii. Infection: Due to inability to retract the foreskin and clean it regularly, dirt and smegma get deposited under the foreskin over the glans penis, which may lead to poor hygiene and chronic infection. iv. Color change: There may be color change around the meatus over the foreskin and over glans penis which looks very bad. Treatment of Phymosis: Can be done by following ways: a) Medical: Is mostly applicable for infants and child. Application of steroid ointment and manual starching of the foreskin can help. However patients present at teen age and adult medical therapy frequently fail. b) Surgical: Surgery is the best form of therapy in any age group. And some condition like pinhole meatus, chronic infection, skin color change, etc best respond to surgery. Stapler Surgery: Here surgery is done by a specialized instrument called circumcision stapler. a) Advantage: It takes only 3 minutes to complete the procedure. No bleeding, no pain and no stitching; the skin margin is held by metallic nails guarded by silicone ring. Skin margin looks perfectly circular, looks very nice and clean. Healing is rapid and complete by 2 weeks. b) Disadvantage: Cost is higher than other two methods. Sometimes the silicone ring may take more than 2 weeks to fall, however it’s not disadvantage, as it eventually falls without any intervention. Hospital Stay: Circumcision is a day care procedure and patient usually stays only 2-3 hours in the hospital. Patient is discharged after 2 hours of surgery with advice of oral medicines for next 5 days. Recovery period: Patient can join in his can ride bike, wear trousers and join in daily duty from the very next day. Patients are prescribed oral medicine for next 5 days for smooth recovery. And it takes about 7to10 days for silicone ring to fall automatically. Sex & Masturbation: After circumcision, patients are advised to avoid sex or masturbation at least for 4-6 weeks. And for 3 months after the surgery, it’s better to us condom during coitus. Hypersensitivity: After circumcision the covering of foreskin over glans penis is absent, so touching and friction with loose garment may cause hypersensitivity sensation. This hypersensitivity goes off gradually by next 4-6 weeks. Diabetes & Circumcision: It has been seen that some diabetic patients frequently suffers from chronic infection of foreskin and glans penis called balanoposthitis. This condition leads to foreskin ulcer, painful skin crack and gradually foreskin become less elastic and phymosis onsets. This condition can be best managed by control of blood and by circumcision.
