Grade 4 Asherman Syndrome | Hysteroscopic Adhesiolysis with cold scissors

This was a 30 year old lady who presented with secondary amenorrhoea since 1 year. No withdrawal bleeding even to estrogen + progesterone. Hormone profile within normal limits. On USG suggestive of thin, irregular endometrium. She had history of D & C thrice done in gravid uterus at 2-3 month POG. Attempt were made to insert copper T, but os could not be negotiated. She was posted for hysteroscopic adhesiolysis. Points to be noted: 1. Follow the path of water 2. Use cold scissors for adhesiolysis 3. Take fibre by fibre snips to be in the correct plane 4. Be slow and steady in dissection. 5. Be patient and the outcome will be superb.
