That is because once the blood supply gets occluded, the more longer the heart becomes ischemic, the myocardium, the muscle in the heart gets damaged. If you can go within first one hour to nearby doctor, hospital. and open up the blood vessel by whatever means either by drugs or by doing a primary PCI then the heart muscles can be saved. That's why we call this first one hour as golden hour and if you go in the first one hour probably most of the damage to heart can be prevented. On the other hand if you think it is gas, neglect yourself, don't go to hospital on time then as you get delayed further the damage to heart increases. If you go after six to 12 hours whatever you do the heart myocardium will be damaged and it won't become normal. That's why all of us should be aware in the first aid like medicine like aspirin, clopidogrel, or ticagrelol nowadays should be taken along with stentin. Go to nearby place and if a place like Apollo where you have primary PCI facilities, we directly take the catheter in, remove the clot and make your blood vessel normal and prevent damage to happening to the heart. If that facility is not there, Tenecteplase is another medicine which can be given in any small clinic also. Safely given like IV injection, it can open the heart blood vessels in 80%. and still prevent damage happening to the heart. So please be aware of this golden hour concept and whenever you have chest pain, don't delay too long, go to the nearest hospital and try to take treatment within one hour so as to prevent damage to heart happening in a long duration. So if you follow this, then probably you can still live a normal life if heart is not damaged after heart attack too. So this awareness of golden hour is most important.
