Hypothyroidism can disrupt normal ovulation, increase prolactin levels (which further inhibits ovulation), and is associated with an increased risk of miscarriage. Optimal TSH levels for conception are generally considered lower than the standard laboratory reference range (often targeted less 2.5 mIU/L preconception and in early pregnancy). Clinical Importance: TSH screening is a standard part of the infertility workup. Identifying and treating even subclinical hypothyroidism with levothyroxine can improve ovulatory function and potentially reduce miscarriage risk.
