Iliotibial Band Syndrome (ITBS): Pain on the outside of knee, thigh and/or hip. Runners, walkers, hikers, bowlers, cyclists, jumpers, sprinters, basketball players, volleyball players and weight lifters are prone to ITBS. Anatomy: The iliotibial band is a long, wide and strong sheath of tough fibrous tissue, much like a ligament, that runs down the outer aspect of the thigh. It originates on the lateral pelvis at the iliac crest. It proceeds down the lateral side of the thigh and knee joint and attaches to the upper outer section of the tibia. The iliotibial band is also known as the iliotibial tract. It has the tensile strength of soft steel and contains very few, if any, stretch receptors. Functions of the iliotibial band include preventing inward rotation of the knee, assisting in knee flexion, assisting in hip abduction and providing stability to the knee and hip. Iliotibial band syndrome (ITBS) is an overuse injury. It occurs when the cumulative effects of repetitive loading bring about excessive tightness in the iliotibial band. Extrinsic and Intrinsic Factors: Numerous extrinsic and intrinsic factors contribute to the development of ITBS. Intrinsic Factors: The chief intrinsic factors include tightness in the iliotibial band, weakness in the medial quadriceps, weakness in the medial thigh, weak core muscles, hyperpronation and weak gluteus medius muscles.
