Radial access with counter-puncture technique for coronary angiography - using angiocath

Counter-puncture technique for radial access in percutaneous coronary diagnostic or interventions. This technique is using long 20G angiocath or IV cath and hydrophilic coated wire. The angiocath is advanced into targeted radial artery until flash backflow of blood into angiocath hub is seen. The angiocath is further advanced through posterior wall of the artery until resistance was felt. The inner needle is removed leaving only the cannula of angiocath. The cannula is then withdrawn millimeter by millimeter until pulsating arterial blood spurt is seen. The floppy hydrophilic wire then inserted into the cannula without any resistance. The cannula is removed leaving the wire only. The 5F or 6F sheath is then placed into the radial artery with the help of the wire.
