Understanding exactly the care manager role, and what is and isn’t a care management activity is really important under Support at Home, the new home care program, especially when it comes to claiming activities against a person’s funding. Many care partners are finding this part of the new program confusing, particularly when everyday tasks like phone calls, follow-ups, rostering, and documentation start to blur together. In this video, we unpack the care partner role in plain English and walk through what actually counts as care management, what doesn’t, and why the distinction matters. Using examples, we explain how care management activities should directly support a participant’s needs, goals, safety and decision-making and why general administration tasks, like rostering or staff replacements, are treated differently.
