This film is about 2 brothers within the same family, Lewis aged 13 and William aged 14, both with the same very rare genetic neurological life-limiting condition. They have very complex needs including tracheostomy, humidified oxygen requirements, reliance on enteral feeding, dystonia, continence issues, dependent for all mobility and personal care needs and both boys have very little, if any, ability to communicate with those caring for them. Both boys have had times of rapid deterioration and have required periods of end-of-life care. They are currently medically stable but with huge care needs and are cared for at home, in a rural area, with limited education home input. Over many years, the NHS funded health care package was commissioned to various care agencies. The care packages have never been fulfilled. There have always been gaps in the shift rotas which the parents, Zoe and Gareth, have had to manage, resulting in their sleep deprivation, extreme exhaustion, physical illness, stress and ultimately some serious safeguarding concerns for the boys. The Children’s Continuing Care team have been integral in thinking about wider solutions with the family and with colleagues in Children’s Social Care, trying to ‘think outside the box’, and prioritise the safety of the children and the emotional and physical welfare of the parents. Both parents have shown remarkable dedication and resilience throughout the years and had the courage to carry on despite the huge cost, physically, emotionally and socially, to themselves. In close collaboration with the parents, the Children’s Continuing Care team now commission the large care packages through Personal Health Budgets (PHB). The team has worked closely with the parents and with other partners such as the brokerage service Enham, to ensure that the PHBs are safe, sustainable and deliver the outcomes for the boys and those that the parents needed to achieve in order to continue to care for their boys without further emotional and physical breakdown. The PHBs are large – both boys have 7 nights waking care and 30 hours throughout the week. There are 13 carers (PA’s) on the payroll for the two boys. This includes the Social Care direct payment workers, who are also trained up and signed off as competent, to ensure that there is cross cover and contingency at all times. There is ongoing bespoke training and support provided to the carers and the family and regular check-ins and phone calls with the parents to ensure that they feel supported as PHB holders. This short film intends to capture the highs and lows of that PHB journey and the often unperceived benefits that can be realised when NHS money is used in a truly personalised way.
