The Rowcroft Hospice At Home Team strives to find out what choices each patient has made and works hard to deliver them. They work with hospital wards and the hospice inpatient unit to get patients back home when time is short and a patient has chosen to be at home. They want their patients to feel that they have some control over this last piece of their life as they feel that this is as important as any other milestone in life. The Hospice At Home Team also see patients that are already at home but need that extra support to enable them to stay there. They collaborate with other organisations and teams to ensure that this transfer of care is as seamless as possible to reduce the stress for the family and provide emotional and practical support for them as well.
