Educational Video. A patient can have chest pain and still a normal ECG as in this case. Patient had a normal ECG but raised troponin levels. Patient had undergone angiography at some other hospital, and underwent angiography there by radial route, and was referred to our hospital for further management due to Triple Vessel Disease and patient refusing for Bypass Surgery. We did a Dobutamine Stress Echo and saw that the RWMA was in the RCA region, and hence patient was taken up for RCA stenting first. Due to cost constraints, and already compromised renal function, complete revascularization was not attempted as that would have required more contrast and increased chances of worsening renal dysfunction and also more radiation exposure. Patient was taken up for PTCA and Stenting to RCA after all possible ✔️ checks according to International Protocols, via femoral route, as radial artery was not palpable after angiography at a different Centre. 2 Stents were implanted with excellent results. Femoral Access was closed with Angioseal device and ambulation was attempted after 12 hours. Patient was discharged after 48 hours of observation in good condition with relief in presenting complaints. The other 2 arteries will be done in a staged manner after 3-6 months or depending on clinical condition thereby preventing a Bypass Surgery for the patient.
