Blood is a critical lifesaving therapy for millions of patients throughout the world. Blood products have always been in critical supply, and the COVID 19 pandemic has exacerbated the shortage. The only source of blood is the generosity of donors. However, the blood products being donated do not always go to the patients; instead, sometimes they go to waste. A certain amount of blood product wastage is inevitable. Because we have an inherent need to maintain a stock of blood products due to the unpredictable demand for those products, and of course, these products have a limited shelf-life. While wastage cannot be eliminated, we believe that it can be lessened significantly through education, inventory management, proper documentation, and oversight of processes. Blood component wastage rates in US commonly run from 1% to 5% of total inventories. Put in perspective, a 1% to 5% wastage rate of the 21 million blood products transfused annually in the United States amounts to 200,000 to 1 million wasted units, at a cost of $46 million to $230 million.1 One of our research studies showed that the total number of wasted blood products in Staten Island University Hospital in 2020 was 425 unit which represents 3.8 % of our total inventories. The total cost of the wastage is $ 97309.46 which does not include the hours spent by the lab personnel for those products. The most common reason and most important contributor/culprit for blood wastage is the Massive Transfusion Protocol (MTP), wherein it is often inappropriately activated, and deactivation is often delayed. Appropriate activation and timely deactivation of the Massive Transfusion Protocol (MTP) is a crucial step toward reducing blood product wastage. In this video, we will discuss our most common causes of blood wastage, with particular emphasis on the MTP and proper techniques and procedures related to the routine blood transfusion.
