www.drscottmaclean.com demonstrates how to change locators on a "planned" NobelProcera Titanium Milled Bar Overdenture. These bars are important to use when too much bone is missing in the upper and/or lower jaw. It makes the implant substructure more stable. Excellent when ridges are small and posterior saddles are not adequate to stop denture fishtailing. Notice that the locators become perfectly level with the occlusal setup by placing them on a bar. Make sure you understand prosthetic space to have the room for your "planned" bar. I say "planned" because in the past many time overdenture bars were a rescue design that was a default after and implant was lost. This solution is for patients that still want to remove their prosthesis but have it sitting on implants instead of implant with tissue borne support. We are "planning" to keep all the implants. There is very little prosthetic movement with this solution. The "All on 4" is still my favourite but this is a great solution for patients that require overdenture features.
