This video is a recording of an abstract presentation prepared for the Canadian Council of Cardiovascular Nurses 2021 National Spring Conference "Update Your Cardiovascular Nursing Toolkit." Enjoy this sneak peak, and consider registering for the full conference through out website. Background. Canadians living with heart failure (HF) deserve equitable access to palliative care. More than 50 000 Canadians are newly diagnosed with HF every year; 50% of whom will die within five years. Disease progression is challenging to predict and marked with periods of symptom exacerbation. With no cure, the goal of treatment is symptom management and complication avoidance. These goals align closely to those of palliative care (PC). The Canadian Cardiovascular Society (CCS) recommends the inclusion of PC for advanced HF management, based on patient needs. Unfortunately, there are many barriers which inhibit Canadians from receiving appropriate PC. Methods. After completing a literature review, PC for HF patients was assessed in light of the CCS recommendations using the policy analysis triangle. Conclusion. Barriers and facilitators impacting the actors, process and context associated with PC for HF patients were identified. In addition, five queues for health care providers working with this population were developed to assist in the appropriate delivery of PC to individuals with HF; train, teach, talk, team, and timing. My presentation will include a brief overview of HF and the importance of PC, the findings of the literature review and policy analysis, and conclude with thoughtful tips to support clinical application.
