AHEAD Model Updates: The group reviewed updates on Maryland’s AHEAD initiative and its implications for physicians and hospitals, ensuring we remain aligned with statewide cost and quality goals. Incentive Payment Methodology: Entities must generate ≥3% savings vs. target to qualify. Tiered shared savings payouts (50%, 65%, 80%) based on efficiency. Quality adjustment applies a 5% withhold, earned back based on performance. Payments capped at 25% of prior-year Part B spend, with final payments issued as lump sums by HSCRC/CRP. Enrollment and Contracting: PY5 enrollment updates shared, including care partner de-duplication and PECOS audit results. All care partners must sign standardized Care Partner Arrangements (via DocuSign) to participate. CRISP will ensure W-9 information matches contracting data to avoid payment delays. Practice Transformation Grant Relaunch: Entities will again have the opportunity to apply for PTG support to enhance workflows, select vendors, and accelerate care transformation efforts. Quality Measures: Final PY3 reconciliation corrected ACP measure coding, and PY5 will only count CPT II codes 1123F and 1124F for the numerator.
