Incentive Payment Methodology: Entities must generate ≥3% savings vs. target to qualify. Tiered shared savings payouts (50%, 65%, 80%) based on efficiency. Quality adjustment applies a 5% withhold, earned back based on performance. Payments capped at 25% of prior-year Part B spend, with final payments issued as lump sums by HSCRC/CRP. Enrollment and Contracting: PY5 enrollment updates shared, including care partner de-duplication and PECOS audit results. All care partners must sign standardized Care Partner Arrangements (via DocuSign) to participate. CRISP will ensure W-9 information matches contracting data to avoid payment delays.
