As we know, prostate MRI is standard imaging in testing for prostate cancer. It can triage patients for when a biopsy is indicated or not. But there’s another reason why reading MRI accurately is so important in prostate cancer, and that’s the emerging practice of focal therapy. Focal therapy for intermediate risk localised prostate cancer requires not only detection of the lesion, but determining its extent, and ensuring there are no other suspicious lesions elsewhere.* So how do we do this safely? Do you even have to have an MRI-visible lesion to perform focal therapy? Conversely, if there is no MRI-visible lesion but there is cancer on biopsy, should it be treated at all? And where does PSMA PET fit into this? If there’s a high suspicion of cancer but the MRI is negative, should we get a PSMA PET? And what about imaging post-focal therapy? When should it be done and do different modalities of focal therapy affect interpretation differently?
