Troubleshooting long-acting insulin: here's my new strategy. I've been experiencing very dramatic stubborn highs nearly every evening for the past couple of months. It left me feeling like I was doing something very wrong — and like I was suddenly terrible at dosing insulin for meals. I need my largest Lantus dose at night before bed because I have the most insulin resistance in the mornings. For reasons I cannot explain, Lantus is no longer lasting 24 hours for me. Now, it's well-known that Lantus doesn't last a full 24 hours for many people but I've never had this issue. (Yes, I've tried Tresiba, Toujeo, and Basaglar — I still prefer Lantus.) Anyhoo, I finally started monkeying with the timing of a teeny dose. I'm taking 20% of my long-acting dose at 2:30pm so it's peaking during those evening hours. (Lantus tends to peak about 5 hours post-injection. People who are very sensitive to insulin notice this peak the most.) And it's working really well! The trickiest part? Remembering to take that little dose in the middle of my work day. I definitely need reminders in place to make it happen. #diabetes #insulin #endocrinology #type1diabetes #type2diabetes
