Introduction • Nursing process is a systematic method for organizing and delivering nursing care. • It can be applied to individual, family (Family Nursing Process) & community (CHN Process). “Family Nursing Process is a problem solving process / approach that addresses family health problems at every aggregate level with the goals of preventing illness & promoting family health”. IV. Elements of Family Health Nursing Process a) Assessment of client’s problem b) Diagnosis of client response, needs thata CH nurse can deal with c) Planning of clients’ care d) Implementation of the care e) Evaluation of the success of implemented care. Assessment Phase • Systematic collection of data • The first phase / initial assessment of family health assessment. Family Health Assessment Elements / Components 1. Establishing a working relationship 2. Assessment of Health Needs 3. Assessment of health problems 4. Assessment of Families 2. Assessment of Health Needs Assessment is a continuous process which becomes more accurate as knowledge of people deepens. a. Family structure, characteristics & dynamics b. Socio-economic & cultural characteristics. c. Home and environment d. Health status of each member. e. Values and practices on health promotion /maintenance & disease prevention 3. Assessment of health problems • Health problems can be identified into three categories i. Health Deficits ii. Health Threats Practices iii. Foreseeable Crisis or Stresses 4. Assessment of Families a) Assessment of environmental condition b) Health status assessment c) Family health practices d) Family lifestyle Assessment of Health Risk Families Health risk families are those who experience a particular event or other events of any diseases repeatedly, that make them more prone towards physical, psychological and environmental response. • Assessment Health risk families Done through : i. Health records ii. Details of Clinics / health centre visits iii. Observation iv. Physical health assessment Assessment Steps: a. Data Collection b. Data Analysis (Actual / potential problems) c. Problem statement / Formulation of family Nursing Diagnosis a. Data Collection • Data collection is the process of gathering information about a client’s health status. • A systematic approach to gather information from the family members to get a complete & accurate picture of the health status of the family. 2. Data Analysis • A process of extracting raw data for the purpose of obtaining constructive information about the family that can be applied to formulating family nursing diagnosis. a. Actual problems and b. Potential problems Family Problems identification • Needs and Problems with Priority List 3. Problem statement / Family Nursing Diagnosis • Once the problem identified through data analysis, it has to be stated in a diagnostic statement. Family Nursing Diagnosis • A clinical judgment about individual or family responses to actual / potential problems. • Diagnosis of health needs & health problems of the family from interpretation of collected data. • A nursing diagnosis has three components / Parts: (1) The problem and its definition (2) The etiology (3) The defining characteristics. 3. Planning (of Client care for Nursing Action • Planning involves decision making and problem solving. • It is the process of formulating client goals and designing the nursing interventions required to prevent, reduce, or eliminate the client’s health problems. Family Health Planning • A systematic process which involves logical decision making at each step of its process. • Based on Family Nursing Diagnosis Family Health Planning Include 4 Steps: 1. Analysis of the diagnosed health problems 2. Establishing Priorities 3. Setting Goals / Outcomes and Objectives 4. Formulation of interventions / family Action Plans Refers to “identification of appropriate / effective family health nursing interventions or strategies & preparing an operational plan to be implemented to achieve the established goals and objectives”. ‘ Family Participatory Approach’ • Making and listing out action plans with family participation. 4. Action Phase / Implementation of Family Health Nursing Process. • Implementation refers to putting the plans or interventions into actions to achieve the set objectives and goals of family health nursing process. Evaluation of the Programme The final step of the family nursing process. “ A systematic process of determining the extent to which objectives are achieved.” Specific steps in Summative / Terminal Evaluation 1. Determine what is to be evaluated. 2. Establishment of Standards & criteria 3. Planning the Methodology 4. Gathering information 5. Analysis of results.
