Ketamine is thought to modulate the effects of glutamate and thereby increases the activity or the effectiveness of SSRIs and SNRIs. We oftentimes recommend that patients don't stop SSRIs and SNRIs when they get ketamine because it may make those medications more effective. Yet, ketamine, by modulating the effects of glutamate in the central nervous system and the brain, essentially may independently, positively modulate the effects of that substance on depression and very much improves depression in the vast majority of our patients. It also works a lot faster than SSRIs and SNRIs, which we know require about four to six weeks to work, sometimes longer. Ketamine can be effective within an hour or slightly more. I've even seen ketamine become effective while a patient's having an infusion after even 30 minutes. So it works completely differently from SSRIs and SNRIs, and it's much more rapid in nature in terms of the way it works and the direction of which it works.
