And this can happen for a few reasons. As you may know, endometriosis is an inflammatory condition and it can cause little adhesions throughout your pelvis. So what this means is that there can be little pieces, almost like scar tissue, that form in between your organ systems. For example, you may have some that are between the bladder and the uterus, or have some directly on the wall of the vagina. For patients who have chronic pain like endo, we tend to clench our jaws, our shoulders, and our pelvic floors. When we're continuing to clench those pelvic floor muscles, that can cause inflammation to the nerves, as well as there being inflammation from the endometriosis itself. Pain in our bodies is really a protection mechanism. It's telling your brain something is wrong, something is going on in your pelvis. So when you start to have a sexual activity, especially a penetrative sexual activity, often our bodies will guard against that and see that as an attack on our pelvic floor that's already inflamed. So what we need to do is learn how to relax those pelvic floor muscles and reduce the pain signals. Oftentimes, folks who have endometriosis will also have other conditions like vaginismus, for example. That's when your vagina will kind of clench closed without you wanting it to during sexual activity or even during gynecological exams. Other folks may experience issues with something called vulvodynia, where the outside of the vagina, the vulva, can be very painful, and that's related to the nerve inflammation in the pelvis. Sometimes just touch with clothing, but certainly touch with sexual contact can flare up that pain.
