Summary: Addressing the constraints in our primary care systems requires fostering strategies, collaborative dialogues, and coordinating activities, as no one health care provider can manage all the primary care needs of individuals alone. A key element of reforming primary care service delivery is the development of team-based primary care that supports comprehensive, coordinated, person-centred care with a focus on ensuring continuity of care across providers and over time. Objectives: -Identify core evaluation domains of team-based care. -Suggest a potential set of metrics for TBC evaluators to use. Judith Krajnak, PhD -Scientific Director, Evaluation, Learning & Impact, Primary Care Alberta -Co-chair, Alberta’s Primary Care Network Evaluator Community of Practice -20 years of evaluation experience including developing the Alberta PHC Evaluation Framework used to guide business planning, measurement and reporting in primary care and chronic disease programming Monica Bull, MSW, RSW -Senior Manager of Primary Health Care, Government of Newfoundland and Labrador -30 years of health care experience, working as a clinical social worker, as a professional practice leader in high-functioning teams in the mental health and addictions sector, and in health care policy and program development Led the development of Family Care Teams: A Health Policy Framework for NL, for the provincial government, released in fall of 2023.
