Point-of-care ultrasound can be used to assess the internal jugular vein to measure the jugular venous pressure (JVP) as an approximation of right atrial pressure. To assess this with ultrasound, start with the patient lying supine in bed and elevate the head of the bed to 30 to 45 degrees. Ask the patient to turn their head to the side, exposing the side of the neck. Using a linear probe parallel to the floor, place the probe just above the clavicle, between the 2 bellies of the sternocleidomastoid muscle to identify the internal jugular vein. With light pressure, follow the internal jugular vein up the neck until it tapers smaller than the adjacent carotid artery. The vertical height of this point is measured in centimeters from the sternal angle, and adding 5 cm to this measurement provides the JVP.
