You asked a great question. You want to know when in the case of a twin pregnancy when both babies may be head down. This actually only happens 42% of the time. 38% of the time one baby is head down and the other baby is in some other position, either breech or head up in the ribs or lying sideways or in some other position, not head down. This tells you that in most instances both babies' heads aren't ever done. Just by sheer fact that there is not much room in there for babies to room around. If your babies are stuck in a certain position at a certain point of pregnancy, meanng later on in the second trimester, beginning of the third, they're probably likely to stay like that unless you've had multiple pregnancies and deliveries before. The uterus gets stretchier with the more babies that you have, it's like an elastic. If this is your first or second pregnancy it's not likely to be very elastic and allow the babies more room to move around. But say it was your seventh pregnancy and you've had lots of kids before, those babies may have a lot more ability to twist and turn and move and change positions up until the day that you're actually going to deliver them, either scheduled or spontaneously. So I have no good answer for you. When it comes time to have your babies, when you go to the hospital the staff there will ultrasound you and find out what positions your babies are in and based on that develop the best plan. The consensus for most obstetricians is that if one baby is head down and the other one is breech it might be safe to try a vaginal delivery but not all obstetricians feel comfortable with that and it also depends on the capability of the facility you're delivering at. If they have staff available that can do an emergency C-section if necessary then the doctor may feel comfortable with that and it also depends on their skills and background. There are other doctors who only feel comfortable doing a vaginal delivery if both babies are head down. I recommend having this discussion with your OB provider. They can talk to you about what their opinions are, what the capabilities are of the facility you're delivering at, and give you the best recommendation based off all of those sorts of things.
