In December 1983, the Coalition was one of the first grant recipients of the Health Care for the Homeless program created by the Robert Wood Johnson Foundation and partners. For four decades, the Coalition has maintained an integrated approach to housing and healthcare in response to the increasing crisis of homelessness and its devastating impact on public health. From the organization’s inception, the principle of integrating housing and healthcare has served as the driving force of CCH’s work. Integration means many things in our work, beyond just clients’ ability to address their healthcare and housing needs through the same organization. At the Stout Street Health Center (a Federally Qualified Health Center) and satellite sites, clients receive integrated medical care, dental care, optometry, pharmacy, mental health care and more. In addition, care teams including case managers, nurses, doctors, medical assistants, and more, work together in an integrated fashion to serve the needs of the whole individual in response to that person’s individualized goals. In this session, CCH direct services staff members will discuss how integrated care, along with a trauma-informed approach and harm-reduction lens are critical in serving those experiencing or at risk of homelessness. CCH panelists include Susan Hudson (Case Manager, DOES), Winnie Promise (Program Manager, HSS), and Karen Spring (Nurse Practitioner). The event will also feature Holly Kingsbury, Quality Initiatives Director from the Colorado Community Health Network to speak about the impact of Community Health Centers statewide. This session is designed for those who want to listen, learn, ask questions, advocate, and engage with important information and proposed solutions to improve the lives of people experiencing homelessness or at risk of homelessness.
