In this CHT Community Story, Lal Kunwar from Medic shares how the Community Health Toolkit (CHT) is transforming child immunization workflows in Nepal, replacing paper-based systems with an end-to-end digital solution that supports health workers and families alike. In many settings, tracking vaccine schedules, calculating doses, and following up with missed immunizations is still done on paper—making it easy for children to fall through the cracks. Using the CHT, health workers in Lumbini Province now manage the full immunization journey digitally—from enrollment to reminders, confirmation, and follow-up. 🔍 In this session, you’ll learn how the workflow works: Child enrollment at health facilities and EPI clinics Automated task generation and vaccine reminders SMS notifications to parents before immunization days Follow-up by community health workers for missed cases Smart closure of services for migration or death This system supports more than 79,000 children, 600 mid-level health workers, and 2,600+ female community health workers across four districts in Lumbini Province—helping ensure timely, complete immunization.
