An additional technique added to the simple continuous suture allows each pass of the suture from the buccal to lingual of the wound margin to have a locking portion. This alleviates the higher likelihood of wound dehiscence if a knot were to unravel early by introducing engaging components at each pass. After each throw from buccal to lingual the loop is left long enough to accomplish a full twist after which the suture needle is passed through prior to cinching the suture throw. Each interlocking knot is approximated off the suture line. Equal bites and equal spacing are important in this technique to avoid tissue ischemia and good approximation of the wound margin. The suture is finished with the same technique as the simple continuous, leaving the last loop long enough to function as the tail for knot tying.
