On today's edition of Thyroid Thursday, we're talking about optimization of T4 and T3. There's a big story in functional medicine that what we need to do to help people feel and function well is to optimize the level of T4 AND T3 in the bloodstream. If we do this, then we've essentially optimized thyroid physiology. I don't think this is the case at all. Let me explain. This is a problem that we struggle with in functional medicine. We see the reduced conversion of T4 to T3 and we automatically want to "fix" it. We assume the body forgot how to do it and in an effort to help our patients, we just give them T3 and hope they feel better. Sometimes it works, but because the reduced conversion of T4 to T3 is an adaptive response (not broken physiology), the initial dose of T3 will help them feel better for a while, but it doesn't last. In this video, I'm sharing a patient example and the patient's lab values. The person was on a dose of medication that I felt was too much, so once her prescribing physician changed medication and we re-ran her labs the next month, there was considerable change. And she experienced a 20% improvement in her symptoms and improved tissue markers. Now, there's still work to be done to identify what's causing the stress response that's reducing T4 to T3 conversion, then eliminate or reduce them, but we are moving in the right direction. This is the strategy I would recommend over just looking to optimize T4 and T3, especially through the use of medication. Let me know if you have questions about this.
