The Stinchfield test or Stinchfield Resisted Hip Flexion Test or the Resisted Straight Leg Raise Test was developed by Dr. Frank Stinchfield. He was a renown hip surgeon and professor who published over 100 articles on hip replacement and arthroplasty. There is no known publication of the test by Dr. Stinchfield but it was written about by Dr. Brian McGrory (McGrory, B. J. (1999). Stinchfield resisted hip flexion test. Hosp Physician, 35(9), 41). He describes this test as a way to determine if a patient’s groin, thigh, or buttock pain symptoms were intra-articular versus extra-articular. These causes could be arthritis, synovitis, occult femoral neck fracture or a prosthetic failure or loosening How to perform the test -Have the patient supine -Flex their hip 20-45 degrees while keeping the knee extended -Apply a resistance force to the thigh A positive is a reproduction of pain in the patient’s groin, thigh, buttock or knee. There may or may not be weakness. There is not much research on this test specifically. One study (Maslowski, E., Sullivan, W., Harwood, J. F., Gonzalez, P., Kaufman, M., Vidal, A., & Akuthota, V. (2010). The diagnostic validity of hip provocation maneuvers to detect intra-articular hip pathology. Pm&r, 2(3), 174-181) did find that it was the most specific test for an intra-articular hip pathology (32%) when compared to FABER (25%), Scour (29%), or IROP (18%). IT only had a sensitivity of 59%. In this study, subjects were tested with these special tests, then had an injection into their hip, if they experienced an 80% relief in symptoms, then it was determined the patient had an intra-articular dysfunction.
