Sands et al. explored how arm positioning affects beam access for upright photon and proton breast treatments. They tested three positions—arms up, arms by the side, and arms back—both with and without a radiotherapy bra. Using optical surface scans, they evaluated breast coverage, field sizes, infra-mammary skin folds, and patient comfort. Results showed radiotherapy bras reduced skin folds and allowed shorter field lengths, though they brought the breasts closer together, increasing planning complexity. The arms-up position offered the best beam access and treatment feasibility, especially with a bra. Arms by the side blocked photon beams, while arms back were unsuitable for some photon plans. Participants reported high comfort and could reliably reposition themselves without assistance, supporting a more empowering, “hands-off” treatment approach.
