Nursing Process 4 basic assessment techniques Process of Assessment Collecting Data Sources of Data Types of Data: – Objective Data: (O-O) Also referred to as signs or overt data, are detectable by an observer or can be tested against an accepted standard. Or those facts that are observable and measurable by the nurse. They are also gathered through diagnostic and laboratory tests. E.g. Patient’s V/s, blood sugar levels, medications used, skin rashes etc. Sources of Data: Primary Source: There is only one primary source, the patient. The patient is the most valuable source because the data that are collected are most current and specific to the patient. Secondary Source: Provides information from some place other than the patient. A family members, medical records of patients, immediate environment of the patient etc. Sources of Data: – Tertiary Source: Provides information from outside the specific patient’s frame of reference. Includes textbooks, the nurse’s experience, etc
