A fever whose diagnosis is not coming forthwith even after 2 weeks is defined as Fever of Unknown Origin. Depending upon the geographical location, ultimately the cause will come out to be either tuberculosis or one of the autoimmune or auto-inflammatory disease. For diagnosis of occult tuberculosis, examination of a 24-hour urine specimen for Acid-Fast Bacilli (AFB) was proposed and studied by Dr, V.M.Bapat. according to his paper published in the JAPI. 2006. (See Reference: https://www.japi.org/r2f47464/mycobac... ). I have diagnosed not less than 850 cases on the basis of positive urine AFB results in 24-hour collection. Autoimmune diseases like Systemic Lupus Erythematosus , Rheumatoid arthritis, Autoimmune Haemolytic Anemia and Auto Inflammatory diseases (Like Familial Mediterranean fever and Adult and Juvenile Still's Disease ---- and the list is growing---) need to be treated with glucocorticoids, and precautions are needed to deal with the possible adverse effects: Hypertension, Hyperglycemia, Hyperacidity and Osteoporosis; these need to be addressed as and when they arise. #Links for other videos of Dr Prakash C.
