(0:44) CN2 - Visual acuity The first step in assessing the optic nerve is testing visual acuity. (1:41) CN2 - Visual fields There are several different screening tests that can be used to assess visual fields at the bedside. (4:01) CN2 – Fundoscopy Direct visualization of the optic nerve head is an important and valuable part of assessing CN2. (4:48) CN2&3 - Pupillary Light Reflex Afferent or sensory limb of the pupillary light reflex is CN2 while the efferent or motor limb is the parasympathetics of CN3. (5:34) CN3,4&6 - Inspection & Ocular Alignment Before checking ocular movements it is important to inspect the eyes. (6:00) CN3,4&6 – Versions Testing extraocular range of motion with both eyes open and following the target is called versions. (6:22) CN3,4&6 – Ductions If misalignment of the eyes or diplopia on versions it is important to examine each eye with the other covered. (7:08) Supranuclear gaze systems – Introduction Supranuclear control of gaze ensures that the image that is being looked at is centered/maintained on the fovea of the retina. (7:26) Saccades Saccades are tested by holding up your two hands three feet apart and asking patient to look at the finger that is wiggling without moving their head. (7:50) Smooth Pursuit Ask to keep watching the target without moving their head. Then move the target slowly from side to side and up and down. (8:13) Optokinetic Nystagmus Optokinetic Nystagmus is a test of smooth pursuit with quick resetting saccades. (8:48) Vestibulo-ocular reflex Vestibulo-ocular reflex is obtained by having patient visually fixate on an object straight ahead, then rapidly turning head form side to side and up and down. (9:20) Vergence Vergence eye movements occur when the eyes move simultaneously inward or outward in order to maintain the image on the fovea that is close up or far away. (9:59) CN5 – Sensory Test for both light touch and pain in the 3 sensory divisions (forehead, cheek, and jaw) of CN5. (10:51) CN5&7 - Corneal reflex Ophthalmic division of CN5 is the sensory limb and a branch of CN7 to the orbicularis oculi muscle is the motor limb of the corneal reflex. (11:39) CN5 – Motor Motor division of CN5 supplies the muscles of mastication. (12:14) CN7 – Motor Motor division of CN7 supplies the muscles of facial expression. (12:57) CN7 - Sensory, Taste Taste is the sensory modality tested for the sensory division of CN7. (13:55) CN8 - Auditory Acuity, Weber & Rinne Tests Vochlear division of CN8 is tested by screening for auditory acuity. (15:05) CN8 – Vestibular Vestibular division of CN8 can be tested for by using the vestibulo-ocular reflex. (15:23) CN9&10 – Motor Motor division of CN9 & 10 is tested by having patient say "ah" or "kah". (15:56) CN9&10 - Sensory and Motor: Gag Reflex Gag reflex tests both the sensory and motor components of CN9&10. (16:09) CN11 – Motor CN11 is tested by asking patient to shrug their shoulders and turn their head against resistance. (16:49) CN12 – Motor CN12 is tested by having patient stick out their tongue and move it side to side.
