This condition arises if fetal red blood cells (RBCs) enter the maternal circulation, typically during delivery, miscarriage, or invasive procedures. The mother’s immune system recognizes the Rh antigen as foreign and produces anti-D antibodies, which can cross the placenta in future pregnancies and attack fetal RBCs, causing hemolysis. This results in hemolytic disease of the fetus and newborn (HDFN), characterized by anemia, jaundice, hepatosplenomegaly, and in severe cases, hydrops fetalis or stillbirth. Rh incompatibility is diagnosed via blood typing, indirect Coombs test, and fetal monitoring with Doppler ultrasound. Prevention involves administering anti-D immunoglobulin (RhoGAM) at 28 weeks of gestation and within 72 hours of delivery or any sensitizing event to prevent antibody formation 💉.
