About this episode: In this paper, the authors challenge the reliance on Body Mass Index (BMI) as a predictor for surgical difficulty in Total Knee Arthroplasty (TKA). This study evaluates whether Local Soft Tissue Thickness (LSTT)—specifically Pretibial Subcutaneous Fat (PSF) measured on standard X-rays—offers a more accurate forecast of implant alignment accuracy than BMI alone. Key Findings: ✅ Superior Predictor: Pretibial Subcutaneous Fat (PSF) was identified as a significant independent risk factor for component malalignment in both coronal and sagittal planes. ✅ BMI Limitations: Surprisingly, BMI showed no significant association with alignment accuracy, whereas increased PSF was strongly linked to deviations. ✅ Clinical Threshold: A PSF threshold of approximately 11 mm can help identify high-risk patients who may benefit from advanced technologies like navigation or robotic assistance.
