I'm going to go through some basic AI searches today to show you where they went wrong, and I'll also give you more relevant information on how to potentially improve your understanding. This is a basic AI search for "sports hernia diagnosis test," so we can figure out where AI went wrong. AI states that sports hernia is diagnosed using a physical exam and imaging such as ultrasound and MRI. I wouldn't necessarily disagree with this. However, AI also says that we need to make sure the doctor checks for a bulge in the groin area and asks the patient to stand, cough, or strain to make the hernia more present. This is not correct. This is an exam for a hernia—an inguinal hernia, an abdominal hernia, or a femoral hernia—but not a sports hernia. In case you're wondering about the differences between the two, we have a full write-up in a free book (linked in the description) that covers this distinction. A sports hernia is more of a muscular injury. It is not an injury where the abdominal contents exit the abdominal cavity. The reason they are called similar names is due to a misnomer given years ago that has persisted. In current literature, you’ll see the term "athletic pubalgia," which is a more accurate description of what we're dealing with. Now, here's another paper that explains why the standard hernia exam is incorrect for diagnosing a sports hernia. This paper covers 245 individuals diagnosed with athletic pubalgia. The commonalities in their physical exams centered around muscle contraction, not coughing or sneezing. What they found was that the adductor contraction test against resistance with an extended knee was positive in 77.6% of the individuals diagnosed with athletic pubalgia (sports hernia). Additionally, simultaneous contraction of the hip in flexion and abdominal flexion against resistance was positive in 76.7% of the cases. We can also see in this paper what similar symptoms people had. Symptoms improved at rest and worsened with sports, running, and walking. Another 13% had pain during sexual intercourse, 11% with sneezing, and 8% with coughing. So, as you see, there is a relationship, but only 11% or 8% of cases had pain with sneezing or coughing. So why is AI saying this is the best test for it? It's not. Once a physical exam has been performed, sometimes advanced imaging is needed, and AI is not wrong about this. AI correctly states that imaging such as X-rays, ultrasound, CT scans, and MRIs can be used to detect injuries to bones, nerves, and soft tissues. Imaging can also help rule out other possible causes of pain.
