Welcome to Chiappetta Shoes Workshop Live! On this little run to the workshop, we're working on a new custom orthotic pick up and copying what we had done a few months back. This customer purchased a second pair for convenience and in this video, we explain a solid modification option for a "Hallux Varus" foot condition. The customer's right foot is a A6 with a #HalluxVarus deformity which is where the metatarsal bones (long mid-foot bones) shift to the outside and the big toe turns in. In cases like these, the foot almost looks like a "C" and can have varying degrees if deformity. To go along with this issue, the customer has a wicked painful #callus on the base of the 5th metatarsal head. She has been using Brown's Foot Bath and that has helped immensely but she still gets stabbing pain when she steps on that spot just right. Modification Details: On the left (happy) side, we just put a simple metatarsal pad with 1/8" cushion cork and a standard tear drop shape. On the more painful right side, we have a "drill & fill" on the 5th metatarsal head... filling with 6mm SR Poron, a T-Form met pad with 3mm multi cork and a cut-out, and a #Corrective 2.5mm medial forefoot wedge with 70 durometer FM EVA. Intentions: On the left side, this #modification is to balance the pressure off so that each side feels similar. The right side's drill & fill is to help offload pressure from that pesky #Callus / #Corn. The met pad is slightly different because this foot is mechanically more sensitive necessitating a lightly softer material, hence the #MultiCork. The shape is more of a broad met-bar, giving more coverage to the forefoot. The medial wedge is key where we are "correctively" steering the hallux forward without rotating medially too much. This wedging has been successful and the material used is usually the starting point. We can go thicker and softer, or firmer with a THK thermoplastic if necessary.
