Erb’s Palsy Explained | Brachial Plexus Birth Injury During Shoulder Dystocia Erb’s palsy is one of the most common brachial plexus birth injuries and often occurs when a baby becomes stuck in the birth canal during delivery, a dangerous situation known as shoulder dystocia. Shoulder dystocia happens when a baby’s shoulder becomes lodged behind the mother’s pubic bone after the head has already delivered. When this occurs, doctors must act quickly and use specific delivery maneuvers—such as the McRoberts maneuver—to safely deliver the baby. However, if excessive traction (pulling) is used on the baby’s head or neck, the brachial plexus nerves in the neck can become stretched or torn. These nerves control movement and sensation in the shoulder, arm, and hand. When the upper portion of the brachial plexus is injured, it can lead to Erb’s palsy, which may cause symptoms such as: • Weakness in the shoulder or upper arm • Limited arm movement • Loss of muscle control in the shoulder • An arm that hangs limp or rotated inward • Difficulty lifting or bending the arm In severe cases, brachial plexus injuries can lead to long-term disability, requiring extensive physical therapy, occupational therapy, and sometimes surgery. Although some children improve over time, many Erb’s palsy cases occur when improper delivery techniques or excessive force are used during childbirth.
