Many people with back pain are told they have a leg length discrepancy—often followed by shoe lifts or inserts. In reality, true structural leg length discrepancies (actual bone length differences) are relatively rare. What I see far more often are functional leg length discrepancies, driven by hip muscle tightness, asymmetrical loading, and myofascial imbalance. When a functional issue is treated like a structural one, symptoms often fail to improve—or sometimes worsen.
