In this discussion, Dr Wirth explains evidence for autoantibodies targeting receptors that regulate blood vessel dilation in people with ME. He outlines how impaired blood flow to muscles and tissues can trigger compensatory processes leading to excess sodium and calcium inside cells, resulting in mitochondrial damage and autonomic dysfunction. This work supports a potentially unifying hypothesis for muscle dysfunction in ME/CFS and in many people with Long COVID and offers a potential explanation for core symptoms such as post-exertional malaise.
